RSI Patient History/Interview

Repetitive strain sufferers, upon posting a question to a discussion group, will often add "Sorry this post was so long," apologizing for providing too much info. But it's actually the opposite problem that occurs, not enough info. This is an unfinished attempt to ask all the questions. Perhaps some day it will be online and in asking for help you can either print it out and hand the paper results to someone, or maybe I'll make some  online technique to copy and paste the results.
